Official Sweepstakes Rules

1. Eligibility:

   – The sweepstakes is open to current and previous patients of Pandia Health.

   – Entrants must be at least 18 years of age at the time of entry.

   – The sweepstakes is subject to all applicable federal, state, and local laws and regulations and is void where prohibited.

2. How to Enter:

 – Eligible individuals may enter the sweepstakes by completing the designated survey provided by Pandia Health or by sending a letter via mail to the address of Pandia Health expressing their desire to enter the sweepstakes. Address to: Renewal Sweepstakes Pandia Health 1257 Elko Dr. Sunnyvale, CA 94089

   – Only one entry per person is allowed, regardless of entry method.

3. Prize:

   – The prize is  ONE (1) $100 eGift card for Target.

   – The prize is non-transferable and cannot be exchanged for cash or any other substitute.

4. Winner Selection:

   – The drawing will take place on July 4th, 2024, conducted by Pandia Health or its designated representative.

   – Winners will be selected randomly.

   – The odds of winning depend on the number of eligible entries received.

5. Notification of Winners:

   – Winners will be notified via email, phone, or mail within a reasonable time after the drawing.

   – If a potential winner does not respond within the specified time period or fails to meet the eligibility requirements, an alternate winner may be selected.

6. Publicity:

   – By entering the sweepstakes, participants grant Pandia Health the right to use their name, likeness, and entry for promotional purposes without further compensation, unless prohibited by law.

7. Release and Indemnification:

   – By participating in the sweepstakes, entrants agree to release and hold harmless Pandia Health, its affiliates, subsidiaries, employees, and agents from any and all liability arising out of or in connection with their participation in the sweepstakes or acceptance, use, or misuse of any prize.

8. General Conditions:

   – Pandia Health reserves the right to cancel, suspend, or modify the sweepstakes if fraud, technical failures, or any other factor beyond its reasonable control impairs the integrity of the sweepstakes.

   – Pandia Health reserves the right to disqualify any individual who tampers with the entry process or violates these official rules.

9. Governing Law:

   – The laws of California govern the sweepstakes.

11. Agreement to Official Rules:
